Creator Bill Lawrence has said in interviews that he wanted "Scrubs" to be almost like a live-action "Simpsons," not only in the style of humor but also with the numerous high-profile guests and a fairly large cast of recurring characters. In a 2006 episode, J.D. commented that "Grey's Anatomy" was so realistic that "it's almost like someone was watching our lives and put it on television," a joke borrowed from an early '90s Simpsons episode saying the same of "Dinosaurs."

Flashbacks reveal the point of view of Ana-Lucia, Mr. Eko, Bernard and Libby and other survivors as they landed on the other side of the island on the beach. As the days progress danger hits their camp as well when "The Others" start taking people against their will. As the survivors race inland, trying to stay a step ahead of The Others, they realize that someone among them wasn't on Flight 815.

This is the first episode with no flash backs to life before the crash. However, since it finishes with the same shot as the previous episode, the entire story is technically a flashback.
